VNS CHOICE offers several quick and easy ways to get more information about our plans, or to enroll in the plan that you would like to join.

How To join VNS CHOICE Medicare Option 1, Option 2 or Option 5 (HMOs):

You can enroll over the phone when you call 1-866-VNS-0047 (1-866-867-0047) or by mail, or we can arrange for a VNS CHOICE Community Liaison to visit you in your home to answer any questions you may have and to complete the application process.

You can also enroll in these plans online.

How to Join VNS CHOICE MLTC and VNS CHOICE MLTC Plus (HMO):

These plans cover long term care services, so the enrollment process requires a health assessment. A VNS CHOICE Nurse will come to your home at your convenience to determine your needs, and to discuss our plans with you and answer your questions.

• Arrange for a home consultation from a Community Liaison, who is a registered nurse
• Request information be sent to you
• Speak with an enrollment specialist
