Online enrollment

VNS CHOICE allows you to enroll online in our Medicare Advantage options. It’s easy, fast—and safe, using our secure online enrollment form.

Online enrollment is speedy and secure. After you have submitted your information, we will send you an email confirming that we have received it. Once we have confirmed your eligibility, you will receive your membership card within 10 days, and you’ll be enrolled in VNS CHOICE Medicare effective the first day of the next month. All enrollments must also be confirmed by the Centers for Medicare & Medicaid Services.

Enrollment in VNS CHOICE MLTC or MLTC Plus requires an assessment by a nurse, who will visit you in your home. For general information or to begin the enrollment process for these plans, please call 1-888-VNS-6555 (1-888-867-6555).

Enrollment by mail

If you prefer, you may also enroll in VNS CHOICE Medicare Option 1, Option 2 or Option 5 (HMOs) by mail. Simply print out a copy of our enrollment form and complete it, then follow the mailing instructions that are on the form.
