The VNS CHOICE Medicare and VNS CHOICE MLTC Plus (HMOs) Provider Directory lists our network of physicians and medical professionals, hospitals, and other facilities.

The VNS CHOICE MLTC and MLTC Plus (HMO) Provider Directory covers our network of long term care providers.

The VNS CHOICE Medicare and VNS CHOICE MLTC Plus (HMOs) Pharmacy Directory lists network pharmacies in the five boroughs of New York City. Note that in addition to the pharmacies listed in the directory, the VNS CHOICE network includes more than 60,000 pharmacies in the 50 states and the District of Columbia.
