Medicaid Long Term Care
VNS CHOICE offers two specialized Long Term Care plans that help our members to live independently in their homes and communities for as long as possible: Managed Long Term Care Plus and Managed Long Term Care (MLTC). The benefits and services of these plans are tailored to meet the needs of people whose needs make them eligible for nursing home care, like New Yorkers with chronic illnesses or disabilities.
