VNS CHOICE MLTC Plus (HMO) is a comprehensive health plan that combines a Medicaid Advantage Plus plan with the benefits of a Medicare Advantage plan.

VNS CHOICE MLTC Plus includes many features specifically designed for individuals who have special health care needs and require supportive services in order to remain at home. This plan combines Medicare and Medicaid together for people who require assistance with day-to-day tasks due to their long term care needs.

This plan covers all of the home-, community- and facility-based care services of VNS CHOICE MLTC, including:

• A Care Manager: When you enroll in VNS CHOICE MLTC or MLTC Plus, you will be assigned a Care Manager (either a nurse or social worker) who will assess your needs and work with you, your family and caregivers, and your physicians to develop a Plan of Care. Your Care Manager will also oversee all of the services you receive and work with your providers to make sure your Plan of Care meets all your needs.
• Home health care and community-based services: We cover professional services (such as nursing services and rehabilitative therapies), as well as personal care and home health aide services in your home (such as assistance with bathing, eating, dressing, toileting and walking). In addition, your Plan of Care may include supportive services from local organizations, such as home-delivered meals and adult day care programs.
• Nurse hotline: If you have a question, a concern—or a potential emergency—you can reach a VNS CHOICE Nurse 24 hours a day, 7 days a week. 
• Transportation to your health-related appointments
• Social and environmental supports (including chore services, home modifications, or respite)

MLTC Plus (HMO) also provides all of the medical and prescription drug benefits of our Medicare Advantage Special Needs plans, including:

• Primary care and specialists,
• Hospital care and emergency care,
• No or low co-payments on prescription drugs, plus coverage through the gap,
• Dental services,
• Hearing exams and up to $1,000 for hearing aids every three years,
• Routine eye exams and $100 toward eyeglass frames and lenses every year,
• Podiatry services, including 4 routine podiatry visits,
• An annual physical.
