VNS CHOICE MLTC and VNS CHOICE MLTC PLUS

VNS CHOICE MLTC is designed for older people with Medicaid who are eligible for placement in a nursing home. VNS CHOICE MLTC Plus is a specialized program for nursing home eligible individuals with both Medicare and Medicaid. You may be eligible to enroll in these plans if you:

• Meet age requirements (18 years old and older for MLTC Plus; 65 years old and older for MLTC),
• Are eligible for Medicaid,
• Are eligible for Medicare Part A and Part B (required for MLTC Plus only),
• Are eligible for nursing home care but choose to remain at home with assistance at the time of enrollment. (A VNS CHOICE Nurse will assess your needs to determine if you need ongoing help with activities such as bathing, dressing, walking, or preparing food.),
• Will require long term care services for at least four months (120 days), as determined by our nurse’s assessment,
• Have your primary residence in one of the five boroughs of New York City,
• Are able to have your health care needs met safely at home,
• Have your enrollment approved by the New York City Human Resources Administration.

Under most circumstances, you are not eligible for any VNS CHOICE Medicare plan, including MLTC Plus, if you have a diagnosis of End Stage Renal Disease. Please contact us for more details.
