Who's in Your Network?

VNS CHOICE MLTC and MLTC Plus (HMO) cover the services you receive from providers who are in our network. Our comprehensive network includes home care services, medical and social service professionals, health facilities and pharmacies in all five boroughs of New York City. You must use plan providers for all of your routine care.

When you join a new insurance plan or when you need home care, you may have two big concerns: Will I still be able to see my current doctors, and are the providers who come into my home trustworthy?

At VNS CHOICE, we understand your concerns. When you join a VNS CHOICE Managed Long Term Care plan, you will receive a copy of the VNS CHOICE MLTC and MLTC Plus Provider Directory. This directory lists providers of all services covered by Medicaid. Every provider has been carefully chosen, and you can be confident that each one—including home health aides, housekeeping and chore services, meal delivery services and therapists—is qualified, insured and has met all requirements for certification or licensing.

We also understand that our members often have long-standing relationships with their physicians, and that switching to new providers who don’t know your medical history can be stressful.

When you join VNS CHOICE MLTC Plus, you will also receive the VNS CHOICE Medicare and VNS CHOICE MLTC Plus Provider Directory, which lists providers of services covered by Medicare. This directory lists nearly 6,000 providers in all five boroughs of New York City, including:

  • More than 5,000 physicians (including dentists and other specialists),
  • More than 40 hospitals, nursing homes, health centers and other facilities.

In addition, our Pharmacy Directory includes more than 61,000 pharmacies, from independent neighborhood stores to national chains. We have also contracted with pharmacies throughout all 50 states, so you can fill prescriptions while you’re on the road. What if your provider is not part of our network?

You may be able to continue to use your current provider until you complete your course of treatment.
