VNS CHOICE Medicare Option 2 (HMO) covers all of the services you receive under Original Medicare, plus Medicare prescription drug coverage. VNS CHOICE also provides several terrific benefits, including a Care Manager who will assist you with all of the medical care you need and help you with other benefits and services for which you may be eligible.

VNS CHOICE Medicare Option 2 (HMO) also offers unique benefits to members, including:

• No or low co-payments on prescription drugs, plus coverage through the gap,
• Free health-club membership, fitness programs and wellness education,
• Comprehensive dental care not covered by Medicaid, including up to $3,000 for endodontic and periodontic services every year,
• Hearing exams and up to $500 per hearing aid (up to $1000 maximum) every three years,
• Routine eye exams and $120 toward eyeglass frames and lenses every year,
• Transportation to and from medical appointments (4 round trips every three months),
• 4 routine podiatry visits,
• An annual physical,
• $1,200 in services toward medical care while traveling outside the United States.

Plus, Option 2 members also receive $65 per month for over-the-counter health products and medications, including antacids, first aid supplies, medications for allergies and colds, vitamins, and many others.
