VNS CHOICE Medicare Option 5 covers all of the services you receive under Original Medicare, plus your Medicare prescription drug coverage. VNS CHOICE also provides several terrific benefits, including a Care Manager who will assist you with all of the medical care you need and help you obtain other benefits and services for which you may be eligible.

VNS CHOICE Medicare Option 5 also offers unique benefits to members, including:

• $5 co-payments for generic drugs and $30 to $60 co-payments for brand name drugs,
• $0 co-payment for primary care, as well as for many other services, screenings, and vaccines,
• $25 co-payment for in-network specialist care, including podiatry, outpatient mental health care, urgently needed care, and outpatient rehabilitation services,
• Comprehensive dental coverage, including up to $1,000 for preventive or restorative services every year,
• Routine eye exams and $120 toward eyeglass frames and lenses every year,
• An annual physical,
• $1,200 in services toward medical care while traveling outside the United States.
