VNS CHOICE medicare option 1 and option 2

VNS CHOICE Medicare Option 1 and Option 2 are only available to people who meet special eligibility criteria. To join either of these plans, you must:

• Be eligible for Medicare Part A and Part B,
• Be eligible for New York State Medicaid,
• Have your primary residence in one of the five boroughs of New York City.

VNS CHOICE Medicare Option 5 is a Medicare Advantage Plan that is available to almost anyone who has Medicare. You may be eligible to join this plan if:

• You are eligible for Medicare Part A and Part B,
• Have your primary residence in one of the five boroughs of New York City.

vns choice mltc plus

VNS CHOICE MLTC Plus (HMO) integrates your Medicaid and Medicare benefits. To join, you must:

• Be eligible for Medicare Part A and Part B,
• Be eligible for New York State Medicaid,
• Have your primary residence in one of the five boroughs of New York City.
• Be eligible for nursing home care but choose to remain at home with assistance at the time of enrollment. (A VNS CHOICE Nurse will assess your needs to determine if you need ongoing help with activities such as bathing, dressing, walking, or preparing food.)

Because VNS CHOICE MLTC Plus is for people who need ongoing help with day-to-day tasks, there are additional eligibility requirements, and enrollment requires an assessment by a VNS CHOICE nurse. To arrange for an assessment, call us at 1-866-VNS-0047 (1-866-867-0047).

Under most circumstances, you are not eligible for any VNS CHOICE Medicare (HMO) plan if you have a diagnosis of End Stage Renal Disease. Please contact us for more details.
