When you enroll in a VNS CHOICE Medicare (HMO) plan, you’ll receive all the services that are covered by Original Medicare (Part A and Part B), plus Medicare prescription drug coverage (Part D) in one health plan—and so much more.
With VNS CHOICE, you get more than 100 years of experience in home health care, particularly in meeting the needs of the elderly and the chronically ill. You’ll be assigned a Care Manager who will coordinate every aspect of your care. Your Care Manager will work with your doctors, help you understand the medications you take and help arrange for any medical services and therapies you need.
Each one of our Medicare plans offers different benefits so you can choose the plan that’s right for you. But no matter which plan you choose, you’ll pay no monthly premiums, no deductibles, and will have no or modest co-payments for the services you receive from our network of providers.
All VNS CHOICE Medicare plans offer complete prescription benefits. Our formulary covers thousands of drugs, including generic, preferred, specialty and brand name medications. Our plans have low co-payments for generic medications, and most of our plan options include prescription drug coverage through the gap.

VNS CHOICE MLTC Plus (HMO) is a specialized program for residents of New York City who have both Medicare and Medicaid, and who have needs that could require placement in a nursing home but would prefer to receive their care at home. This plan fully integrates the benefits of our Managed Long Term Care (MLTC) plan and a Medicare Advantage plan into one comprehensive program.
