VNS CHOICE offers four Medicare (HMO) plans. Option 1 and Option 2 (HMOs) are Medicare Advantage Special Needs plans. These plans are designed for people who are eligible for Medicaid as well as Medicare.

In 2010, VNS CHOICE Medicare is introducing Option 5 (HMO), a Medicare Advantage plan that is available to almost anyone in NYC who has Medicare.

VNS CHOICE MLTC Plus (HMO) is a Medicare Advantage Special Needs plan that incorporates Medicare and Medicaid services.

All VNS CHOICE Medicare (HMO) plans have no monthly premium and no deductibles, and cover services obtained from our network of providers. No referral is required.
