You get more than just insurance coverage when you join a VNS CHOICE health plan. All of our plans provide extensive benefits—including prescription drug coverage—as well as excellent customer service and a Care Manager who will help coordinate all aspects of your care to help you manage your medications and get all the services you need.

VNS CHOICE offers five health plans: three Medicare Advantage plans, one Managed Long Term Care plan, and one specialized plan that combines the benefits of both. Each one of our plans offers specific benefits to make it easy for you to select the one that best meets your needs.

No matter which plan you choose, you will receive:

• Care management. As a VNS CHOICE member, you’ll be assigned a Care Manager who will oversee your care. Your Care Manager will work with you, your caregivers, and your doctors and other providers to coordinate all of the services you need, make sure your medications are right for you and help you understand how to take them, and help you connect with resources in your community.
• A health risk assessment. When you enroll in a VNS CHOICE plan, your Care Manager will complete a clinical assessment of your individual health care needs. Based on your medical needs, we may offer additional services to help you stay as healthy as possible.
• Nurse hotline. A VNS CHOICE nurse is available 24 hours a day, 365 days a year, to answer your health questions or to help you get the services you need—when you need them.
• Customer service coordination. It can be confusing to understand which services you are eligible for and which program pays for which benefits. VNS CHOICE staff help you coordinate your services, so you can be sure you’re receiving all of the care you’re qualified to receive.
• Coverage for prescriptions. VNS CHOICE plans provide coverage for your prescriptions. Our Medicare plans have no or low co-payments for generic prescription drugs, and others with modest co-payments for medications. As a VNS CHOICE member, you may qualify for extra help to pay for your prescription drug premiums and costs, and you may receive coverage for your prescriptions through the gap. 
• Worldwide coverage. As a member of a VNS CHOICE Medicare plan, your benefits will follow you when you travel around the country or around the globe.

When you enroll in a VNS CHOICE plan, you’ll pay no monthly premiums and no deductibles. Most of our plans require no co-payments for medical services, while some require modest co-payments.
